For social workers, discharge planners, and care navigators

A free, credential-verified directory you can route families to without endorsing anyone in particular. Built so you can do less searching and more case management.

What this is

  • Every provider listing is verified at the source regulator (RHRA, LSO, OHCA, etc.) and re-checked on a fixed cadence.
  • We accept no money for placement — providers cannot pay to rank higher or appear at all.
  • We log every change in a public audit ledger.

What this means for you

  • Routing a family to a verified provider doesn't make you their advocate; it's information, not endorsement.
  • You can hand families a one-pager spec sheet (PDF, printable) per provider for a clean handoff.
  • We surface what we did not check (cost, quality, fit) so the burden of evaluation stays with the family — see our limitations page.
  • The triage wizard at /about/triageroutes families through a 3-question funnel — point them there if you don't have time to walk them through options yourself.
